Informed Consent/Hold Harmless: I give permission for my child to participate in the East Coast Eagles Baseball. (hereinafter, the “Team”) practices, games, and activities. I understand that participating in the Team events may involve risks and dangers, both known and unknown, and have elected to allow my child to participate in these events. Therefore, I voluntarily accept and assume responsibility for all risk of injury, loss of life, or damage to property arising out of participation in the Team’s tryouts, practice, games, events and other activities. I further agree to indemnify, hold harmless, release, discharge, and covenant not the sue or seek damages of any kind from the Team, the Team’s Board of Directors, Officers, Staff, Employees, Coaches, Assistant Coaches, Trainers, Advisors, Agents, other participants, officials, advertisers, sponsors, and owners and lessors of the premises and facilities used for Team events and activities, from any and all liability, damages or costs of any kind as to any right of action that may accrue to me or my heirs, assigns or representatives for any injury to my child or loss that my child may suffer while participating in any Team activity. I hereby certify that I am authorized to sign this form on behalf of my child even if my child is 18 years old or older. All players, if 18 years old and older, also agree to the terms of this paragraph.
